Enrollment Procedures - Joint Enrollment for NALE 6/23/08

1. Enroll with NOLS

• First, fill out 4 NOLS enrollment forms
  • Application (pdf)
  • Medical Form (pdf)
  • Risk Acknowledgement (pdf)
• Insurance Form (pdf)
• Would you like to apply for a scholarship?
  • Scholarship Form (pdf)
  • Please complete the Scholarship Application and submit it to NOLS by January 31, 2008.
• Please complete the Application Form and return it to NOLS along with a $60 registration fee as soon as possible. You can mail or fax forms to:

• Your enrollment is finalized only after all enrollment forms have been received and reviewed, and the course tuition and equipment deposit have been paid in full.
• NALE 6/23/08 is the only joint enrollment course for summer 2008.
• Deadline for receiving paperwork and tuition:
